As a place-based private nonprofit foundation, we advance racially equitable health outcomes in our area by improving the social drivers that shape them. We lead, fund, partner, and advocate to improve health, change systems, and strengthen our community.

Problems are put into perspective by data. Equity data and analysis help illustrate the scale and impact of issues, can point to new connections, and ensure the solutions we invest in are doing the work they promised to do.
