An Introduction to Healthy Equity

Racial discrimination has a wide-ranging and enduring legacy in Pinellas County, Florida. Social determinants are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. The Foundation uses a social determinants of health framework to prioritize race equity, improve population health and achieve equitable health outcomes throughout the geographic area we serve. By paying attention to the root causes of inequality in our community, the Foundation is able to target investments, convening, policy and communications work where they can have the greatest impact.
