Name, Image, or Likeness Policy – Universal Release

Please be advised that attending or participating in events and or activities of the Foundation for a Healthy St. Petersburg and the Center for Health Equity may result in the use of your name, image and/or likeness in printed and electronic material, including but not limited to publication on the World Wide Web, social network sites, in press materials, and in advertising and market materials.

The Foundation may capture photography and/or recordings on Foundation property and during Foundation events to use in its publicity materials and on its websites and social media sites.

By entering the event premises, you specifically waive and release the Foundation for a Healthy St. Petersburg and its agents from any and all claims arising from your name, image and/or likeness based on any of the uses stated above.

All visitors consent to the use of their photo or recording taken on Foundation premises or during Foundation-sponsored events unless they specifically inform staff of an objection to such use prior to or while such photographs or recordings are taken. No names will be utilized in conjunction with photos without express written consent.

Community Meetings and Events

Community organizations holding scheduled events or meetings in Foundation meeting rooms or spaces may arrange for their own photography and/or recordings of their event. Such photography and recording are restricted to the space reserved by the group and the people in the group. Moreover, the group is responsible for any necessary releases and permissions.

Questions or Concerns

Guests who have a concern about the use of their image or would not like to be photographed, please contact a member of the Foundation for a Healthy St. Petersburg Staff upon entering the premises.
